A Subsidy Health and Safety inspection was conducted on 12/14/2017 to review supplemental health and safety requirements. The inspector arrived at 7:30 A.M. and departed the program at 10:00 A.M. There were ninety children present with twenty direct care staff members. During the inspection all eleven classrooms were observed, breakfast time was observed, infant care was observed, diapering was observed, hand washing was observed, nap time was observed, emergency supplies were viewed, eight medications along with authorizations were reviewed, policies and procedures were reviewed, and a sample of injury reports were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on review of eleven staff's records, the center failed to ensure that all time frames for completion of First aid and CPR and the preservice training was completed. Evidence: 1. Staff # 3 DOH was 8/23/2017 and on the day of the inspection did not have the required first aid and CPR training. The training is required within 30 days of employment. 2. Staff # 5 and 7 did not have then required 10 hours of preservice training that was required within 90 days of employment. DOH for staff # 5 was 8/31/2017 and DOH for staff # 7 was 8/2/2017.

Plan of Correction: The director will have the staff to obtain the required training ASAP.
