22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted from 10:30am-1:30am At the time of entrance, 131 children were in care with 26 staff members present. Children were observed during outdoor play, toy play, handwashing, lunch. A selection of 8 staff and 14 children records, evacuation drills, and other documents were reviewed. Standard #: 22VAC40-185-130-B
Description: Based on a sample review of records, 1 child under the age of two was missing documentation of immunizations once every six months. Evidence: The most recent documentation of immunizations in the record for Child #2 [23 months old] were obtained 1/31/2017. Documentation of additional immunizations would have been required on or by 7/31/2017.

Plan of Correction: We will email and request the information from the parents.

Standard #: 22VAC40-185-140-A
Description: Based on a sample review of records, 1 of 14 children's records was missing documentation of a physical examination within 1 month of enrollment. Evidence: The record for Child #4 [date of enrollment: 9/11/17] was missing documentation of a comprehensive physical examination.

Plan of Correction: We will request this information from the parents.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of records, 1 staff was missing central registry results [or the equivalent] by any other state in which the individual has resided within the previous five years. Evidence: Staff #4 [date of hire: 10/18/2017] documented living in Missouri within the past five years on their sworn disclosure statement. There was no documentation that a central registry request has been submitted to that state within 30 days of the staff's employment.

Plan of Correction: We will initiate the background check request.
