A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 2:45pm-4:20pm. There were 27 children in care directly supervised by 4 staff. A sample of 4 children's records, 3 staff records, and 2 medications were reviewed. The children were observed eating snack, doing homework, and enjoying staff lead crafts. One new staff member has been hired since the last inspection. There were no violations founded during this inspection. The exit interview was conducted with the Director. If you have any questions, I can be reached via e-mail at Kimberly.Sawyer@dss.virginia.gov or by phone at 571-835-0599. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
