Comments:
An unannounced monitoring inspection was conducted on December 14, 2017, between the hours of approximately 9:55 a.m. and 3:30 p.m. Fifty-three (53) children were enrolled and nineteen (19) children were in the care of four staff members, in three classrooms, during the on site inspection. Two additional staff members were present and assisted, as needed. Six children's records and six staff records were updated and/or reviewed. Three children's topical ointments, five children's medications, accompanying authorization forms and one medication administration log were reviewed on this date. Safety equipment (first aid kit, flashlight, electrical outlet covers and radio), posted information (license, inspection documentation, weekly menu, emergency evacuation diagrams/plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill/shelter-in-place drill records, attendance records, one local agency report and injury reports) were observed/reviewed. The daily schedules accurately reflected the activities provided. The lunch served on this date included ravioli, green beans, applesauce and milk. Observed staff/child interactions were positive and supportive of children's development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Children's artwork was displayed. Program observation was completed. Activities observed on this date included: indoor play and learning - grouptime (flash cards), stories, songs, coloring pictures of candy canes, playdoh and toys; handwashing and restroom procedures; lunch; naptime; arrival and pick-up/departure of children. Previously cited violations (inspection date - 5/16/2017) were reviewed for compliance; no repeat violations were cited. Exceptions to compliance were noted on the current violation notice. Standard #: 22VAC40-185-70-A
Description: Based on review and/or update of six staff records and discussion with staff #1 on December 14, 2017, the program failed to obtain verification of age and document completion of orientation training for one staff member. Evidence: 1. Review of staff #7's record, on this date, determined that verification of age and documentation of orientation training were not included in the staff member's record. 2. Staff #1 verified the lack of required information in the staff member's record.

Plan of Correction: Verification of age was obtained from the staff member on December 14, 2017 and completion of orientation training was added to the staff record.

Standard #: 22VAC40-185-510-G
Description: Based on review of five children's medications and discussion with staff #1 on December 14, 2017, the program failed to ensure the current prescription label, in accordance with the prescriber's instructions, was obtained for one child's medication. Evidence: 1. Review of child A's medication, on this date, revealed that the prescription label and the date on the box did not match the current prescription medication. The prescription label on file was for an expired medication. 2. Staff #1 confirmed that the prescription medication on site had a current date but the prescription label and accompanying box had an expired date.

Plan of Correction: The child's parent was immediately contacted and a current prescription label will be obtained for the medication.
