A Subsidy Health and Safety inspection was conducted on December 15, 2017 to review supplemental health and safety requirements. Upon arrival at 11:55 AM there were a total of twenty-one (21) infant and pre-school age children present under the direct care & supervision of three caregivers. Adult to child ratios were not in-compliance upon arrival. Licensing Inspector was unable to review staff and children records; records were requested but not provided. LI Anderson observed children during arrival/departure procedure, rest/sleeping activity, bathroom/hand washing procedures, snack/meal served, group activities, self-initiated activities and homework/quiet time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-005-B Description: Based on inspection & discussion with the Center Director, the Licensee failed to maintain for each group of children, a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. EVIDENCE: For each group of children, the Center does not document the arrival and departure of each child in care as it occurs. Plan of Correction: Attendance will be taking the Staff in each class at the time of arrival and departure.
Standard #: VENDSUB-000-006-B-11 Description: Based on inspection, the Licensee failed to have a written allergy care plan for each child with a diagnosed food allergy. EVIDENCE: Allergy care plan was unavailable for 5 children. Licensing Inspector observed the center's posted allergy which included 5 children with food allergies. Plan of Correction: Director will get documentation from the parents
