A Subsidy Health and Safety inspection was conducted on 1/10/18 was conducted from 12:20 pm-1:40 pm to review supplemental health and safety requirements. At the time of inspectors' arrival there were 4 infants in care with 2 staff members. Children were observed participating in infant activities, interacting with staff, lunch, transitioning between activities, and nap. A sample of 5 children's records and 2 staff records were reviewed. Medications are not being administered at this center. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements are detailed on the Violation Notice and were discussed with the Provider at the conclusion of this inspection.
Standard #: VENDSUB-000-005-B Description: Based on record review it was determined the center did not maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The sign in/out sheets were not fully completed. Plan of Correction: Staff will ensure that the time in and out is completed for each child upon arrival and departure.
