Comments:
An on-site investigation began on 1/10/2018 in response to a complaint being reviewed at our office. Interviews were conducted and observations were made. The preponderance of evidence supports the allegation and the complaint is deemed valid. The areas of non-compliance can be found on the Violation Notice. Standard #: 22VAC40-185-570-F
Description: Based on observation, heated bottles were not tested for temperature before serving to children.
Evidence: At the time of the investigation, Child #1 and Child #2 were given bottles and the temperature of the bottles were not tested prior to being given.

Plan of Correction: We will re-train our staff on bottle feeding.
