Comments:
An unannounced monitoring inspection was conducted with the director beginning 10:00 AM and ending at 2:16 PM. There were 62 children, ages 3 months through 5 years old, in care during the inspection. The children were observed playing with manipulative, being read a story, and singing songs. The children were observed eating lunch which consisted macaroni and cheese with meat crumbles, broccoli, peaches, and milk. Children were also observed during nap time. Ten children?s records and five staff records were reviewed and in compliance. Five medications were reviewed. The first aid kit, flashlight, and weather band radio were observed and in compliance. The emergency escape drill log was reviewed and in compliance. The most recent emergency escape drill was documented as December 13, 2017. The most recent shelter in place drill was documented as April 6, 2017. The most recent lock down escape drill was documented as April 6, 2017. The most recent fire inspection was documented as August 2, 2017. The most recent health inspection was documented as September 30, 2017. Violations:
Standard #: 22VAC40-185-80-A
Description: Based on review of the attendance record, the facility did not ensure all children were included on the written attendance record. Evidence: There were 12 children physically present in the Preschool A classroom and 15 signed in on the attendance sheet. There were 14 children physically present in the Preschool B classroom and 11 signed in on the attendance sheet.

Plan of Correction: The center will update attendance sheets for children that transitioned to a different classroom.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain equipment inside the center in clean and operable condition. Evidence: One crib mattress in the infant C classroom had a tear on the side making it absorbent.

Plan of Correction: The center will replace the torn crib mattress with a new one.

Standard #: 22VAC40-185-510-G
Description: Based on review not all the medications had a written authorization from a parent or guardian to administer prescription medication. Evidence: Child A's (DOB: 2/1/2014) and Child B's (7/10/2012) records did not have written authorization from a parent or guardian to administer prescription medications.

Plan of Correction: The center will obtain signed authorization forms from the parents as soon as possible.
