A Subsidy Health and Safety inspection was conducted on 1/22/18 1to review supplemental health and safety requirements. An unannounced renewal inspection was completed 1/22/18. There were 39 children present with three staff directly supervising, several volunteers, the program administrator, and the program director present. Five children's records and five staff records were reviewed. No medications were at the center. The inspector arrived at the center at 2:00 pm and departed at 6:15 pm. The children were observed during arrival, during snack, during homework time, and during gym time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on record review, the center failed to ensure that the required Virginia Pre-service training was completed by all staff. Evidence: 1. No staff had not completed the VA Pre-service training. Staff hire dates were as follows: #1 - 6/15/14; #2 - 2/10/15; #3 - 2/17/16; #4 - 2/27/17; #5 - 9/20/17. Plan of Correction: Staff will complete this training.
