Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out of state central registry forms. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Each facility must establish a point of contact with DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligible letters will be sent out by emails and staff may not work with children (ratios or supervision) until a completed letter is received. Please read, review and respond to all correspondences regarding the fingerprint roll out.

Comments:
An unannounced monitoring visit was conducted today with the preschool and the playgroup. The preschool group had 8 children with a staff member, parent co-op member and a staff from TR assisting. Children were observed in free play, transition to rest room, hand washing, preparation for outside and active play on the playground. Room is well equipped. The playgroup has 9 children present with a staff member and 8 parents present. Children were observed eating a snack of fruit and cheese and crackers, free play and hand washing. Emergency preparation is up to date for both rooms. Areas of non-compliance are identified in the violation notice. If you have any questions regarding the inspection, please feel free to contact me. Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 3 children's records were incomplete. Evidence: Child #1 and 2 in the preschool group were missing proof of identity documentation within 7 days of attendance.

Plan of Correction: Site Director will reach out to the parent and registration to obtain the information.

Standard #: 22VAC40-191-60-B
Description: Based on record review, the revised Sworn Disclosure Statement (SDS) has not completed correctly. Evidence: Staff hired or volunteering after July 1, 2017 must use the revised SDS and the individual used the older version that did not ask if the individual has lived outside the State of VA within the past five years. She completed the form 10/2017 (playgroup).

Plan of Correction: The new SDS form will be used moving forward to ensure all information is obtained.
