Discussed current changes to background checks.
Today an unannounced Monitoring Inspection took place between the hours of 9:40am and 1:40pm. At the start of the inspection there were a total of 35 children with 6 staff; all ratios were incompliance. A total of 5 children?s files and 5 staff files were reviewed. The children were observed having their diapers changed and bottles fed, being read to and learning about different shapes they observed outside. The center managers stated that there are currently no medications on site. If you have any questions or concerns, please feel free to contact me at firstname.lastname@example.org. Standard #: 22VAC40-185-160-C Description: Based on observation, the center did not obtain a tuberculosis (TB) test for one employee every two years. Evidence: Staff #4 most recent TB screening on file is dated 8/5/2015. Plan of Correction: She has an appointment tomorrow.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain street addresses for both emergency contacts. Child #2 file did not contain the child or parent's addresses or street addresses for both emergency contacts. Plan of Correction: I'll get this information tonight when the parents pick-up.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas of the center were not maintained in a safe and operable condition. Evidence: 1. The foot pedal on the trash can in the two's classroom has broken off, leaving a piece of medal protruding from the bottom of the can. 2. The foot pedal on the trash can in the three's classroom does not work. As a result, the children and the staff are required to lift the lid of the trash can with their hands to throw out trash. 3. There is a large storage bin on the back play area that is broken, creating sharp plastic edges that are exposed. 4. There is a large gate that is off the hinges and leaning up against the fence that divides the back play area and the preschool playground. 5. There is a small purple and green slide on the toddler playground that has a broken step with a large piece of plastic protruding out from the step. 6. All climbing structures on the preschool playground have large areas of peeling paint and rust where the children would put their hands to climb. Plan of Correction: We will fix these issues as we can. We will throw out equipment and work on the playground when the weather is better.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1. There was a spray cleaner in the cabinet under the changing table in the infant classroom. There was a lock on the cabinet, however the lock was hanging from the handle, unlocked. 2. There is hand sanitizer on the sink in the infant classroom that is accessible to the children. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning materials were stored above food. Evidence: There are several cleaning agents stored on the shelf above infant food (purees and puffs) in the infant classroom. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-500-A Description: Based on observation, proper hand washing was not practiced as required. Evidence: A child was observed having their diaper changed in the toddler classroom. The child did not have their hands washed after being changed, and the teacher did not wash her hands prior to picking up another child to have their diaper changed. Plan of Correction: I'll have a talk with all the teachers that are changing diapers about washing their's and the children's hands.
Standard #: 22VAC40-185-570-E Description: Based on observation, infant bottles are not labeled with either a child's name and/or today's date. Evidence: In the infant classroom, there was one bottle with no date, three bottles with no name or date, and five bottles with the incorrect date. Plan of Correction: We will remove all tape is removed from bottles at the end of the day when they are washed. Teacher's will label bottles daily as the children come in each day.
Standard #: 63.2-1720.1-B-3 Description: Based on record review and staff interview, the center allowed staff to continue employment and supervise children without obtaining an out-of-state central registry check from outside the Commonwealth of Virginia. Evidence: Staff #3 (date of hire: 9/20/2017) sworn disclosure statement confirmed that the employee had lived outside of the state of Virginia in the last five years. Center management did not submit the required out-of-state central registry check. Plan of Correction: We will have this employee fill this out when she comes back to work.
