Today's visit was a routine monitoring inspection. The inspection began at 9:15am and concluded at 4:20pm. There were eighty-two children present with seventeen staff members. During the inspection classroom observations, lunch time was observed, infant care was observed, diapering was observed, ten staff records were reviewed, ten out of one hundred forty-three children's records were reviewed, emergency supplies were viewed, nap time was viewed, the emergency plan was reviewed, four of eight medications were viewed, and some additional policies and procedures were reviewed. Standard #: 22VAC40-185-130-B Description: Based on the inspector's review of eight out of one hundred forty-three children's records, the center failed to ensure a child's record had an immunization update every six months when the child was under two years of age. Evidence: Child #7 was enrolled in the program on 11/5/14 and the child was thirteen months of age. The child's immunization record reflects the most recent immunizations for when the child was six months of age. Immunizations are required evey six months for children under the age of two. Plan of Correction: The PD contacted the parent during the day of the inspection to request a copy of the child's most recent inspection.
Standard #: 22VAC40-185-50-A Description: Based on the inspector's observations in the toddler room, children's allergy, food preferences, and sensitivities were posted on a wall in a public manner. Evidence: The inspector observed a list of children's names with information regarding food allergies/sensitivities/intolerences/and food preferences next to the sink in the classroom. This information is considered confidential unless otherwise authorized by the parent to be posted in a public manner. Plan of Correction: The center will ensure the information is removed and posted in a manner that is not accessible to the general public.
Standard #: 22VAC40-185-280-B Description: Based on the inspector's observations in the toddler classroom, the center failed ensure a hazardous substances was kept in a locked manner. Evidence: The inspector observed a large bottle of hand sanitizer on the counter at the children's sink. The container stated "keep out of reach of children" and had multiple additional warnings. The hand sanitizer is required to be locked. Plan of Correction: All hand sanitizer will be kept in a locked manner.
Standard #: 22VAC40-185-380-A Description: Based on the inspector's observations in the Junior-K classroom, the center failed to ensure a daily schedule was posted. Evidence: The inspector conducted an observation in the Junior-K classroom and there was no daily schedule posted for the classroom. A staff member stated due to some changes it was in the process of being updated. Plan of Correction: A daily schedule will be posted by the end of the program day.
Standard #: 22VAC40-185-450-D Description: Based on the inspector's observations in the Junior-K, the center failed to ensure a pillow with a case was assigned for individual use. Evidence: The inspector observed a pillow within a case that was not assigned in any manner for individual use. The pillow and case were located with a rest mat. Plan of Correction: The pillow and case will be labeled with the child's name before nap time. Staff will be reminded to assign linens and pillows as they come into the center if not alresdy done so by the parent.
