22VAC40-191 Background Checks (22VAC40-191)
This unannounced monitoring inspection was conducted from 10:10 am until 2:00 pm. At the time of the inspection 21 children were present with five staff. The sample size consisted of four staff's records, five children's records and two children's medications. Children and staff were observed during free indoor and outdoor play, snack, lunch preparations, hand washing, lunch, art activities, and nap. Standard #: 22VAC40-185-290-3 Description: Based on observation, the staff failed to ensure all electrical outlets did not have protective covers. Evidence: In the PreK classroom an electrical outlet under the wet dry board did not have a protective cover on one of the outlets and the power strip under the wet dry board did not have a protective cover on one of the outlets. Plan of Correction: Will be corrected immediately. The classroom teacher is absent today and wold have had these covered.
Standard #: 22VAC40-185-340-D Description: Based on record review and interview, the director failed to ensure each group of children have at least one staff member who meets the qualifications for program leader.
1. The records for staff 1 and 4 were reviewed and neither staff have the qualifications to be a lead teacher per the standards. Both staff's records state they are PS Extended Day Coordinators.
2. The director states there are two classes that serve different age groups for the PS Exended Day program and staff 1 and 4 are the lead teachers for both the classrooms.
Plan of Correction: The director will meet with both staff members today and make plans for meeting the standard within 30 days.
Standard #: 22VAC40-185-500-A Description: Based on observation, the staff failed to ensure children's hands were washed with soap and running water or disposable wipes before and after eating meals or snacks. Evidence: The children in the Preschool classroom were observed during the end of snack . Three children who had not finished snack at the time of the observation did not wash their hands or use wipes after eating. Plan of Correction: The director will review this requirement with all program staff today.
Standard #: 22VAC40-185-510-A Description: Based on medication review, the administration failed to obtain parent authorization to administer emergency medication for one out of two children. Evidence: 1. The documentation for the emergency medication (Epi-pen & Benedryl) for child MAT 2 did not contain documentation of parent permission. 2. The administrator that hands the medication stated the parent works at the center and they failed to obtain permission from the parent. Plan of Correction: Administration will have this corrected today.
Standard #: 22VAC40-185-560-K Description: Based on observation, the staff failed to ensure tables were sanitized before children ate at the table. Evidence: In the PreK classroom children were observed getting ready for lunch. The three tables used by children during lunch were not sanitized before lunch. Plan of Correction: A substitute teacher was on duty and this was missed. The classroom teacher sanitizes the tables before each meal.
Standard #: 63.2-1720.1-B-3 Description: Based on record review and interview, the director failed to obtain an out of state child abuse/neglect search within 30 days of hire for two out of four staff. Evidence: 1. The record for staff 1, start date 8/30/17,did not contain a an out of state child abuse/neglect search. According to staff 1's record she has lived in another state within the last five years. 2. The record for staff 4, start date 8/30/17,did not contain a an out of state child abuse/neglect search. According to staff 4's record she has lived in another state within the last five years. 3. The director verified an out of state child abuse/neglect search has not been completed for staff 1 and 4. Plan of Correction: The director is submitting the verification forms today.
