Discussed documenting first date of attendance in children's records rather than relying on attendance sheets.
Today an unannounced Monitoring Inspection took place between the hours of 9:40am and 11:30am. At the start of the inspection there were a total of 16 children with 4 staff; all ratios were incompliance. A total of 3 children?s files were reviewed. Staff files are reviewed on a regular basis at central location in Leesburg, VA. The children were observed working on connect-the-dot letter activities, building with sugar blocks, playing in small groups and other organized activities. The children provide their own lunches and the scheduled snack for this afternoon is cheese sticks. If you have any questions or concerns, please feel free to contact me at firstname.lastname@example.org. Thank you for your cooperation during this inspection.
Standard #: 22VAC40-185-510-A Description: Based on observation, the center did not obtain a written authorization from a parent to administer a medication that was being kept on site. Evidence: Child #4 had an emergency medication stored in a locking device in the classroom in the event of an emergency. There was no written authorization for this medication to be administered. Plan of Correction: We will start from scratch to get our medication forms in order.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
