Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.
The following inspection took place between the hours of 11 am through 1:30 pm. Today, I met with the Staff of the Lovettsville Montessori School located in the town of Lovettsville, Virginia. At the start of this unannounced monitoring inspection there were a total of 35 children (25 in child care and 10 with the elementary program) with 4 staff; good ratios in all both rooms. A total of 6 children?s files and 5 staff files were reviewed. The children were engaged in the following: outside play on the center playground, learning activities, centers within the classroom, bathroom breaks, hand washing, lunch, Japanese cooking, and other organized activities. Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 6 children's files reviewed indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after attendance. Evidence - 1. On the date of inspection (01/30/2018), a physical examination was not available for Child #5. 2. The start date for Child #5 was listed as - 08/30/2017. Plan of Correction: Documentation of a physical examination shall be obtained from the parents of Child #5 and placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review and interview, 1 of 5 staff files reviewed indicated that not all of the required information/documentation was contained within each staff record. Evidence - 1. The Director stated (during an interview conducted on the date of inspection - 01/30/2018) that 1 of 2 references (as to character and reputation as well as competency) have been obtained for Staff #5. 2. Based on the information on file, the start date for Staff #5 was listed as 01/06/2017. 3. This same standard was cited previously during an inspection conducted on 09/07/2017. Plan of Correction: An additional reference shall be obtained and placed on file for Staff #5.
