Comments:
Today an unannounced monitoring visit was conducted with the Acting Director. This visit took place in the morning between 8:10am-10:20am there were 57 children with 12 staff within the supervision guidelines. The children were observed having morning breakfast, free play, story-time, blocks, circle time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, fire drill log, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-270-A
Description: Based upon observations made, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) 2 bottles of disinfectant were found inside an unlocked cabinet in the main hallway outside of the Preschool classroom. 2) Small holes were found & chipped paint found in the bathroom outside of the preschool classroom. 3) Cracks along the tile were found near the exit door in the toddler classroom. 4) Holes were found along the cabinet wall near the exit gate in the toddler classroom leading to the 2's classroom. 5) THE floor tile/equipment in all of the classrooms had black stains/marks and need to be cleaned.

Plan of Correction: Acting Director stated that Corporate is currently in the process of replacing all of the floors and a work order will be put in for all holes and cracks.

Standard #: 22VAC40-185-280-B
Description: Based on observed of the physical plant, cleaning agents were found in a unlocked cabinet accessible to children. Evidence: Cleaning agents were found with a safety latch unlocked in the main hallway near preschool classroom.

Plan of Correction: Acting Director stated that Verbal Coaching was done with Staff during this inspection about making sure that cabinets are locked immediately after use.

Standard #: 22VAC40-185-400-B
Description: Based on interviews and observation, staff were not observed redirecting children in a manner constructive in nature, age and stage appropriate. Evidence: Upon arrival in the school age classroom, Staff #1 was observed "nudging" in attempt to move the child. Licensing inspector observed what appeared to be staff attempting to zip/help child with coat and while doing so Staff #1 was "talking at the child in a manner that wasn't constructive in age " and afterwards the child was nudged in a attempt to move the child. On 2/1/18, Staff #2 was observed "flicking a child's ear while attempting to redirect a child from taking a crack from another child.

Plan of Correction: Acting Director stated that Staff #2 was put on administrative leave effective immediately. Acting Director stated that they will have some verbal coaching and some positive guidance training for Staff #1.
