Comments:
Licensing Inspector arrived at 10:00am and departed at 12:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 53 children in care with 16 staff present. Children were observed on the playground, participating in dramatic play and during lunch. A sample of 3 staff and 6 children's records were reviewed. The center had no medications at the time of inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a record review the center failed to ensure each staff member submitted documentation of a negative tuberculosis screening submitted no later than 21 days after employment. Evidence: Staff #2 (doh 03/25/15) did not have documentation of having tuberculosis screening.

Plan of Correction: The program director provided the following corrective plan of action during the exit interview: The staff member had a TB test done however did not have documentation and the clinic closed; therefore there are no records. Director will send staff out tomorrow for a screening.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: On the preschool playground there was peeling paint on the poles of the shaded structure and the hardware on the composite structure was rusted.

Plan of Correction: The program director provided the following corrective plan of action during the exit interview: During the inspection the director placed a work order to fix the items on the playground.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances werekept in a locked place using a safe locking method that prevents access by children. Evidence: In the second infant room there was a liquid air freshener plugged into an electrical outlet.

Plan of Correction: The program director provided the following corrective plan of action during the exit interview: During the inspection the director removed the air freshener from the classroom. The director reminded the staff about how to store hazardous substances.
