- Technical Assistance:
Today we discussed meal service, changes in background checks, fingerprints and background check compliance.
An unannounced monitoring inspection was conducted from 10:30am through 1pm. There were 13 children in care with three staff. Children present ranged in age from 17months through five years old. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including play, restroom routines, lunch and transitions. Violations were observed in meal service and background checks and appear on the violation notice.
Standard #: 22VAC40-185-560-F Description: Based on observation and interview the center failed to ensure that when meals are served the center will follow the most recent age-appropriate nutritional requirements of a recognized authority such as the USDA. Evidence: 1 ? The meal served to the children in care does not meet the USDA requirements as a half cup of box macaroni (mac) and cheese was served to each child in the toddler group. Children present in this group ranged in age from 17 months through 3 years. a. The box of mac and cheese states that there are 8 grams of protein in each 1 cup serving of the product. b. When asked staff 2 stated that the children in the toddler group were served 1/2 a cup of mac and cheese. 2 ? When asked staff 5 was unable to demonstrate knowledge of the amount of protein each child is required to be served. 3 - Staff 5 was not able to state how much protein each child was served. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-191-40-D-1-C Description: Based on record review and interview the center failed to obtain a criminal history record check before three years since the date of the last criminal history record check report. Evidence: 1 - The record for staff 4 contains a criminal history record dated 1/15/15 and was due for updated before 1/15/18. a. There is no documentation that a criminal history record update was requested in the staff record. 2 - Staff 4 confirmed that the background check was past due for update. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 63.2-1721.1-E Description: Based on record review and interview the center failed to ensure that no person be involved in the day-to-day operations of the child day center or be alone with, in control of, or supervising one or more children, without first having completed any required background check. Evidence: 1 - Staff 3 was observed alone caring for children ages 3 to 5 years old throughout the inspection. a. The record for staff 3 contains a criminal history background check dated 1/30/15. Therefore the background check expired on 1/30/18. 2 - Staff 4 confirmed that the criminal history record check has expired and neither a criminal history check nor fingerprint check had been requested prior to the expiration of the criminal record check on file. Plan of Correction: Not available online. Contact Inspector for more information.
