Comments:
A Subsidy Health and Safety inspection was conducted on 2/8/18 from 10:05am to 12:10pm to review supplemental health and safety requirements. At the time of entrance there were 17 children ages two years old to five years old in care with four staff. Children were observed participating in various activities in the classrooms. Records were reviewed for five children five staff . A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The attendance record for the Pre-K classroom had not been completed for 2/5/18 - 2/8/18. 2. Staff #3 and staff #4 both confirmed that the attendance record for the Pre-K classroom had not been completed this week.

Plan of Correction: The facility responded: Pre-K teacher had been out sick plus no one was assigned to take the roll. We will make sure to take roll. Vice Principal will monitor, Principal will ensure.
