*Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. **VA Preservice training can be counted as annual training one time. Please notify your inspector of suspension or termination of services for more than 24 hours as a result of an emergency. Please notify your inspector, via online reporting form, of any serious injuries sustained at the facility. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). There are currently 3 staff employed by the facility. All staff have documentation of the mandatory VA Preservice Training and are currently certified in first aid and CPR. There is one child with diagnosed food allergies currently enrolled - please request an allergy action plan. The emergency preparedness plan was reviewed for reunification plans and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Standard #: VENDSUB-000-006-B-11 Description: Based on review of three children's files, the facility failed to have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: there was no documented allergy action plan for Child B, who has a diagnosed food allergy. Plan of Correction: Center director will request an allergy action plan for Child B.
