An unannoucned monitoring inspection was made on 2/9/18. There were 25 children present with four staff supervising. Five children's records were reviewed. Four staff records were reviewed on 2/15/18 and the inspection was completed on that date with the child care director. No medications at the site. The inspector arrived at the school on 2/9/18 at 3:00 pm and departed at 5:00 pm. The children were observed during snack, during group games in the gym, and during electronic/toy time.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child had a physical examination completed prior to attendance or within 30 days after attendance. Evidence: 1. There was no physical examination in the record for child #1. Child #1 first date of attendance was 1/5/18. Plan of Correction: The staff will talk with the parent to get permission to get a copy of the child's physical examination from the public school office.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all children's records contained documentaiton of all required information. Evidence: 1. The record of child #3 was missing the following required information: the address for two of two required emergency contacts for the child, work place and work phone number for the mother and father of the child. Plan of Correction: The staff will have the parent complete the information today at pick up.
Standard #: 22VAC40-185-560-K Description: Based on interviews, the center failed to ensure that tables used for snack were sanitized as required. Evidence: 1. Two staff who were interviewed stated that the tables were wiped with disinfectant wipes before snack. This was the only step that is taken to sanitize the tables. The Standards require that the tables are sanitized by spraying the tables with a disinfectant solution. Plan of Correction: Staff will ensure a spray disinfectant is used before and after use for snack in the future.
