Comments:
A renewal inspection was the conducted with the director and area manager 40 Children were in care during the inspection with four staff. The age range of the children was five to ten years old. Four staff and ten children records were reviewed. Two staff were current in MAT. The inspection time was 3:30pm-5:35 pm

Violations:
Standard #: 22VAC40-185-360-A
Description: Based on observation, the center did not provide children with a variety of daily activities that were age and stage appropriate and provide opportunities for teacher-directed or self-directed activities. Evidence: LI observed the children from 3:30pm to 5:35 pm during this time the children were not offered any group or staff lead activities. The staff placed paper and crayons on the tables in which the children showed no interest in.

Plan of Correction: Zones are being implemented in the afternoons during homework time. This is provided to the children who do not have homework.

Standard #: 22VAC40-185-400-B
Description: Based on observation, the center did not provide behavioral guidance that is constructive in nature and age and stage appropriate to redirect children to appropriate behavior. Evidence: The licensing inspector observed Staff A frequently yelling, sometimes from across the gym, at the children in harsh and loud voice tones to redirect them or guide their behaviors.

Plan of Correction: Staff is being retrained in positive behavioral redirection

Standard #: 22VAC40-185-510-A
Description: Based on medications review and director interview, the center did not have written authorization to administer medication to a child in care. Evidence: There were no written medication authorizations for Child A's Epipen.

Plan of Correction: Written authorization was located in the back of medication binder after inspection.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the center did not prevent children from drinking or eating while walking around. Evidence: The licensing inspector observed three children eating snacks while walking around the gym. A fourth child was drinking from a juice box while walking around.

Plan of Correction: Children were reminded of rules and expectations.
