An unannounced monitoring inspection was conducted this date. A total of 95 children with 13 staff were on site today. Observation of all 10 classrooms was conducted. Staff interviews and physical plant were completed. Children were observed transitioning from lunch to nap time. The KG classroom were reviewing the weather and anticipating the weather for tomorrow. A total of 5 staff records were reviewed and 10 children's records were reviewed. Records were found complete and well organized. All medications were reviewed and authorizations were determined complete. This inspection began at 11:55am and ended at 3pm. Thank you for your cooperation today. Standard #: 22VAC40-185-70-A Description: Based on review of staff records, documentation of a lead qualification was not found. Evidence: Staff #1 is a designated lead teacher. No documentation found in the staff record of lead teacher qualifications. Plan of Correction: We will obtain this documentation from this staff.
Standard #: 22VAC40-185-370-5 Description: Based on observation of the infant room 1, one infant was not offered a change in play space after 30 minutes. Evidence: One infant was observed being placed in a chair device at 12:20pm and remained in that chair play space until approximately 1:00pm. Plan of Correction: We will remind staff of repositioning infants to a new play area every 30 minutes or as needed.
Standard #: 22VAC40-185-500-A Description: Based on observation of the Intermediate A classroom and bathroom procedures, hand washing was not completed as required. Evidence: Licensing Inspector observed Staff 1 assist 3 different children with toileting and changing soiled clothing. Staff #1 did not wash hands before and after assisting each child with changing soiled clothing and diapers/pull ups. Plan of Correction: We will re-train all staff of the hand washing procedures and will continue to monitor.
