Technical Assistance:
Discussion on the center's own topical ointment authorization forms and the state model forms. Discussion on what is considered soft bedding and stuffed toys which are not allowed in cribs when infants are sleeping.

An unannounced non-mandated inspection was conducted this date. A total of 49 children with 11 staff were present during the inspection. Observation of all 5 occupied classrooms was made. Children were observed finishing lunch and transitioning to nap time. lunch today was chicken, rice, peas and pineapple. Two medications on site at this time. Children in the Octopus classroom were observed completing a table top coloring activity. Observation of bathroom procedures and handwashing was made. A total of 5 children's records and five staff records were reviewed and found complete. A new director of record, qualification for this position was verified. Thank you for your cooperation today. Standard #: 22VAC40-185-370-1
Description: Based on observation of the Gold Fish infant room and staff interview, one infant is placed on their stomach to sleep. Evidence: Child #7 was observed sleeping on their stomach. Staff stated that this child has a medical necessity that requires them to be placed to sleep on their stomach. The center failed to obtain physician documentation to allow the child to be placed to sleep on their stomach.

Plan of Correction: Requested Drs. note for child to sleep on stomach due to medical reasons.

Description: Based on observation of the Octopus class and staff interview, the classroom was not equipped with a diapering area. Evidence: This classroom does not have a diapering area. One child in this classroom requires diapering. Staff was observed changing this child while the child was standing up.

Description: Based on review of the center's medications, one medication was not properly labeled. Evidence: Child #6's Epi Pen was not found in the original container with the pharmacy label.

Standard #: 22VAC40-185-560-F
Description: Based on observation of the physical plant, the current menu was not posted on the second floor of the center. Evidence: The menu posted outside of the Octopus class was for the month of January.

Plan of Correction: Curren menu posted on utilized classrooms bulletin boards

Standard #: 22VAC40-185-560-K
Description: Based on observation of the Octopus classroom, areas used for eating were not sanitized. Evidence: The tables located in the classroom were not properly sanitized prior to the children eating lunch.

Plan of Correction: Trained teachers on proper sanitation guidelines.
