Comments:
An unannounced monitoring visit was conducted today. All groups are in ratio today. Children were observed washing hands, eating lunch, and watching a movie. The menu, first aid kit, medication and emergency escape drills are all in compliance. 2 staff and 3 children's records were reviewed. See violation notice form missing items. Violations:
Standard #: 22VAC40-185-160-A
Description: Based upon a review of staff records, TB screening was not on file within 21 days of employment for 2 staff members. Evidence: Staff A (Date of Hire (DOH) 6/15/17) Screening is dated 9/13/17 Staff B (DOH):10/27/17) Screening is dated 1/30/18.

Plan of Correction: We will not have staff start until they have a TB screening done.

Standard #: 22VAC40-185-60-A
Description: Based upon a review of records, Children's records do not include all required information. Evidence: 2 of 3 records reviewed did not include complete addresses for emergency contacts. (Children 1,2)

Plan of Correction: Parents will be asked to provide the missing information.

Standard #: 22VAC40-185-360-A
Description: Based upon direct observation, children's activities were not age and stage appropriate. Evidence: Children age 2 years- 4 years were watching a PG rated movie at LI arrival.

Plan of Correction: All PG rated moves will be removed from the facility.

Standard #: 22VAC40-185-550-C
Description: Based upon direct observation, the emergency escape maps do not contain all required information. Evidence: Missing information includes, a secondary escape route and the shelter in place location are not denoted.

Plan of Correction: The maps will be updated.
