The licensing inspector conducted unannounced complaint inspections on 1/26/2018, 1/30/2018 and 2/1/18 in response to two complaints received by the licensing office on 1/22/18 and 1/23/18. The complaint allegations were inadequate staff-to-child ratios and inadequate supervision of a child. After interviews were conducted on 1/26/18, 1/30/18 and 2/1/18 with the appropriate staff and taking staff-to-child ratios at the center, the information gathered supports the allegation of inadequate supervision of a child. The complaint is determined to be valid. Please complete the columns for Plan of Correction and Date to be Corrected for each violation cited on the violation notice, and return a signed and dated copy to the licensing office within five (5) business days of receipt (2/16/18). Complaint related: Yes
Description: Based on staff interviews, review of documentation the center did not ensure a child's care, protection and guidance. Evidence: On 1/22/18, at approximately 1:00 p. m. child #1 (3 years old) who was described as a "runner" by six staff ran away from staff #1 during nap time. Staff #1 was supervising 17 children ages 3-5 years at nap time (Attendance Roster shows 16 children plus child #1) with no assistance. Child #1 ran from the back of the building down the main hall pass the french door and exiting the building using the front door before being stopped approximately four feet outside of the building by staff #2. Staff #2 reported that after she stopped child #1 she needed to use the door-code to re-enter the building. Staff #2 stated that she was on her lunch break when the incident occurred and that she just happened to be sitting on the bench in the hallway when she heard staff #1 yelling for help. The french door by the office area had been left open leaving access to the front door of the building. There was no available staff in the office or in the center to stop child #1 from exiting the building or assisting staff in such an emergency.

Plan of Correction: We are respectfully requesting a desk review of this violation in a separate written request.

Complaint related: Yes
Description: Based on interviews with staff and review of documentation, the center did not have a staff member who met qualifications of program leader with one grouping of children. Evidence: On 1/22/18, the center administration stated that all staff that were present between 12:30-1:30 (naptime) were either on their lunch break, or in the classroom to cover for lunch breaks. 1. Staff #3 did not meet program leader qualifications. 2. Staff #3 was left alone with a grouping of 17 children ages 3-5 years during naptime. 3. The exception requirements for not having a program leader in each grouping of children during the designated rest period were not met. 4. Staff #3 does not have at least three months of programmatic experience at the center.

Plan of Correction: Ensure a staff member meets qualifications for naptime/solitary supervision of children and ensure 1 extra staff member available outside of ratio in the building at naptime to assist in emergency.
