Comments:
On February 13, 2018, between the hours of approximately 10:45 a.m. and 3:45 p.m., an unannounced monitoring inspection was conducted. Seventy-three (73) children were enrolled and nineteen (19) children were in the care of five staff members, in five groups/two classrooms, during the on site inspection. Seven additional children (school-age) arrived for care at 3:45 p.m. Two additional staff members were present and assisted, as needed. Six children's records and five staff records were reviewed and/or updated. Five children's medications and two children's diaper ointments were observed/reviewed on this date. Emergency equipment (first aid kit, flashlight and battery-operated radio), posted information (license, inspection documentation, weekly menu, emergency plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drills/shelter-in-place drill records, attendance records, infant daily records, injury report forms, emergency plan, and local agency inspection reports) were observed/reviewed. The daily schedule accurately reflected the activities provided. Lunch served on this date included: cheese ravioli; peas; peaches and milk. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities provided on this date included: indoor play (toys, songs and movement); diapering, restroom and handwashing procedures; lunch; naptime; infant care and arrival/departure of children. Standard #: 22VAC40-185-160-A
Description: Based on review of five staff records and discussion with staff #1 on February 13, 2018, program staff failed to obtain documentation of a tuberculosis (TB) screening completed by one staff member within 12 months prior to or 21 days after employment. Evidence: 1. Review of staff member #3's record, on this date, revealed that the TB exam/screening submitted was completed on 8/15/2016. The staff member's date of employment was 10/23/2017. 2. Staff #1 confirmed the date of the TB exam/screening and the staff member's date of employment.

Plan of Correction: An updated TB exam/screening will be completed and documented.

Standard #: 22VAC40-185-320-B
Description: Based on observation, measurement of the water temperature and discussion with staff #1 on February 13, 2018, the program failed to ensure the water temperature at the bathroom sink, in the toddler/2-year-olds section of the building, did not exceed 120 degrees F. Evidence: 1. As measured and observed, on this date, the water temperature at the bathroom sink, in the toddler/2-year-old section of the building, measured 127.8 degrees F. 2. Classroom staff confirmed the water temperature.

Plan of Correction: The thermostat on the hot water heater will be turned down. The water temperature will be monitored to ensure it does not exceed 120 degrees F.

Standard #: 22VAC40-185-510-G
Description: Based on review/observation of five children's medications and discussion with staff #1 on February 13, 2018, program staff failed to obtain written parent authorization to administer one child's medication. Evidence: 1. Review/observation of child H's medication and accompanying written parent authorization form, on this date, determined that the authorization form gave permission for the child's public school, not the child day center staff, to administer the medication. 2. Staff #1 verified the content of the written parent authorization form for child H's medication, including the agency authorized to administer the medication.

Plan of Correction: The child's parent has been contacted and written permission will be obtained for the child day center to administer the child's medication.
