Comments:
Today an unannounced monitoring inspection was conducted between the hours of 2:15pm and 4:20pm. I observed 46 children directly supervised by 4 staff. Ratios and supervision were in compliance. Children were observed arriving to the program, doing homework, playing table games, participating in group play and eating snack. Today's snack consisted of CapriSun Berry 100% juice and Colby Jack cheese cubes. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Description: Based on review of medications, the center has a medication on-site that doesn't match the written medication authorization form. Evidence: Reviewed the medication records for child#1, and found that the written medication authorization form authorized an Epi-Pen, but the medication on-site is an Avi-Q.

Plan of Correction: I will ask parents to either get a new or updated authorization form.

Standard #: 22VAC40-185-510-F
Description: Based on observation, 1 of 6 medications were not maintained in the original container with the direction label attached. Evidence: Child#1 has a non-prescription medication that is not in the original container with the direction label attached.

Plan of Correction: We'll contact the parent to request the original box.
