Comments:
Licensing Inspector arrived at 9:45am and departed at 2:15pm to conduct an unannounced monitoring inspection. At the time of the tour there were 79 children in care with 16 staff present. Children were observed playing with toys, during circle time and being read a story. Lunch was also observed. A sample of 5 staff and 8 children's records were reviewed in addition to 2 medications.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a record review the center failed to ensure each child had a physical examination under the direction of a physician. Evidence: Child #5 (doe 12/13/17) did not have documentation of having a physical examination.

Plan of Correction: The director provided the following "Plan of Correction" during the exit interview. The director has given the parent 2 weeks to get the physical to the center. If the physical is not provided within the 2 weeks, the child will be disenrolled.

Standard #: 22VAC40-185-60-A
Description: Based on a record review and interview with the program director, the center failed to ensure all required information was kept for each enrolled child. Evidence: The program director confirmed Child #1 (doe 09/08/17) is no longer enrolled in the center. The child's file did not have documentation of the child's last day of enrollment.

Plan of Correction: The director provided the following "Plan of Correction" during the exit interview. The director recorded the child's last day in the record during the exit interview.

Standard #: 22VAC40-185-320-B
Description: Based on observation the center failed to ensure each restroom for children met all requirements. Evidence: The bathroom in the 2 year-old room did not have toilet paper. There were 2 dispensers that were observed with empty rolls in them.

Plan of Correction: The director provided the following "Plan of Correction" during the exit interview. Staff in the classroom added toilet paper to the dispensers during the inspection.

Standard #: 22VAC40-185-440-H
Description: Based on observation the center failed to ensure cribs met all requirements. Evidence: In the infant room, child #4 was observed sleeping in a crib that had more than 1 inch of space between the mattress and the crib one corner of the service side.

Plan of Correction: The director provided the following "Plan of Correction" during the exit interview. The director will inspect the crib and evaluate. The director will ensure the crib has no more than 1 inch of space between the mattess and the crib when a baby is sleeping.
