Comments:
A Subsidy Health and Safety Inspection was conducted on February 20, 2018 to review supplemental health and safety requirements. The director was present for the inspection. In the Infant/Toddler room there were 8 children sleeping with 2 staff supervising. In the Preschool Room there were 29 children sleeping on mats, and they were being supervised by 2 staff. Two school age children were also present with 1 staff.
Five children's files and 2 staff files were reviewed. Evacuation drills had been conducted and were documented.
A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-11
Description: Based on a review of a child's record, the center had not obtained a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The written information for Child # 6 indicated that the child had a food allergy. There was no written allergy care plan on file for the child.

Plan of Correction: The parent was notified today and told that the child could not return until the plan was on file.

Standard #: VENDSUB-000-012-B
Description: Based on a review of documentation, staff did not complete the first 90 days of employment or subsidy vendor approval the Virginia Preservice Training for Child Care Staff. Evidence: Staff # 1, hired 10/12/17, and Staff # 2, hired 9/6/17, did not have documentation of completion of the Virginia Preservice Training.

Plan of Correction: Staff will complete the training by 3/2/18.

Standard #: VENDSUB-000-012-E
Description: Based on a review of documentation, the center did not ensure that all staff providing care to children had completed first aid and CPR training within 30 days of employment. Evidence: Staff # 1, hired 10/12/17, and Staff #2, hired 9/6/17, had not completed first aid and CPR training.

Plan of Correction: The training will be completed by 3/2/18.

Standard #: VENDSUB-000-031-A
Description: Based on a review of the emergency preparedness plan, the center did not have written procedures for reunification of children with their parents in the event of evacuation, shelter in place, and lock down scenarios.

Plan of Correction: The reunification procedures will be on file with the plan.
