Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on February 23, 2018 from 11:10 a.m. through 12:20 p.m. Upon arrival, there were fourteen (14) children in the care of two (2) staff. Five (5) children's and three (3) staff records were reviewed and updated. No medication is being administered at this time. Areas observed were: free choice play, story-time, hand washing/bathroom procedures library and dismissal. Interactions between the staff and children were observed as positive and the children seem comfortable with their daily routine and schedule. Standard #: 22VAC40-185-160-C Description: Based on record review three staff did not have documentation of obtaining an updated tuberculosis screening. Evidence: 1. Three staff records were reviewed. Staff #1, #2 and #3's last documented tuberculosis screening was documented on 08/13/15. 2. The director confirmed updated had not been obtained for Staff #1, Staff #2 and Staff #3. Plan of Correction: Staff will obtain an updated tuberculosis screening as soon as possible. Documentation will be sent to the licensing inspector.
