Discussed the storage of sunscreen.

Comments:
An unannounced renewal inspection was initiated today at 12:30pm. The physical plant, 5 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 24 children under the supervision of 4 staff. During the inspection, children were observed reading books, listening to music, preparing for and resting during naptime. For lunch, the children were served soft tacos with beef (or chicken) and cheese, mixed vegetables, oranges and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 4 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-130-A
Description: Based on file review, 1 out of 5 children's records does not have an immunization record before the child received care at the center. Evidence 1. Child #5 does not have an immunization record on file.

Plan of Correction: We will ask the parents for this.

Standard #: 22VAC40-185-160-C
Description: Based on file review, 2 out of 5 staff records do not have a TB test/screening completed within the past two years. Evidence 1. Staff #4 and #5 have their most recent TB test/screening having been completed on 2/10/15.

Plan of Correction: They will complete this.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 5 out of 5 children's records do not have all required information. Evidence 1. Children #1-5 do not have the work phone number and place of employment of each parent. 2. Child #5 does not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: The administrator will work on obtaining this information from parents.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 5 staff records does not have all required information. Evidence 1. Staff #5 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment. There is only one reference check on file.

Plan of Correction: The second reference check will be completed.

Standard #: 22VAC40-185-270-A
Description: Based on observations, there was an area in the center that was not maintained in a safe condition. Evidence 1. In the 3's classroom, there is a section of the wall, near the floor, where the paint is chipping off.

Plan of Correction: This will be fixed.

Standard #: 63.2-1720.1-B-3
Description: Based on file review, 1 out of 5 staff records does not have all required background checks. Evidence 1. Staff #1 does not have a search of the central registry from a state in which the individual has resided in the preceding five years.
