Comments:
The Licensing Inspector conducted an unannounced monitoring inspection on January 25, 2018. The LI observed 30 children in care. The children were separated into 2 classrooms. Each group of children was supervised by two staff members. As reported, 36 children are currently enrolled in the program. A sample of 6 children's records was reviewed. Documentation of the center's annual fire and health inspections was current. The children were observed by the LI as they participated in varied activities to include nap/rest time, snack, hand washing procedures, free play, music and dismissal at the end of the program day. Staff were engaged with the children. The LI reviewed the center's revised emergency plan and the center's documentation of the annual review of the injury prevention procedures. Prescribed medications were observed in the Center. Multiple staff have completed medication administration training. The physical plant was clean, orderly and furnished with age appropriate supplies and materials. The front room of the Center has been transformed to appeal to the pre-K children and to increase interest in areas such as arts and science. Findings of the inspection were reviewed with the Center Lead Teacher at exit. If you have any questions, please call this LI at (540)309-2310. Standard #: 22VAC40-185-140-B
Description: Based on a sample review of children's records, the Center failed to ensure documentation of a child's physical examination completed prior to attendance was within the prescribed time period: within 12 months prior to attendance for children between two years of age through five years of age. Evidence: Documentation of the physical examination form for child 6. was not completed within the 12 month time frame. The child was four years old at time of registration, date of entry was noted as 10/02/17. The physical exam assessment date was noted as March 4 2016.

Plan of Correction: The responsible staff will notify the child's parent and request documentation of a current physical examination form. Staff will review documentation closely to ensure physical exams are current at time of registration.

Description: Based on review of children's records, the center failed to ensure required information was complete in each record. The following information was not available for review: A.3. work phone number and place of employment of each parent who has custody; and A.15. documentation of viewing proof of the child's identity and age Evidence: To include a child's record which was previously reviewed at the July, 2017 inspection, a sample of 6 children's records was reviewed. The record of child 1. did not contain documentation to verify proof of the child's identity and age, this is a repeat violation. 2. The work phone number for the father's place of employment was not noted in the record of child 2.

Plan of Correction: The responsible staff will review each child's record to ensure information is obtained and complete in each record.
