An unannounced monitoring inspection was conducted on March 1, 2018. There were 45 children present with six staff members in three classrooms and one volunteer. The children were observed napping during the inspection and those not resting were lying quietly on their cots or assisting their teachers. One medication was reviewed during this inspection. Five children's records were reviewed during the inspection. The outdoor play area was not inspected due to inclement weather.
Standard #: 22VAC40-185-140-A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: Before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on March 1, 2018, there was no documentation of a physical examination in the record for Child #2 (start date 9/5/17) Plan of Correction: The physical will be requested by the parents or the nurse will be contacted and the child may not be able to return without it.
Standard #: 22VAC40-185-60-A Description: Based on record review, two of five children's records reviewed, the licensee did not ensure that each child's record contained all required information. Evidence: During the inspection conducted on March 1, 2018, the following information had not been documented in each child's record: Child #1 and Child #3-viewing proof of the child's identity Plan of Correction: The parents have been contacted to send the information.
Standard #: 22VAC40-185-260-A Description: Based on observation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on March 1, 2018, the fire inspection report for the facility was dated 9/26/16. Plan of Correction: The fire marshall has been contacted twice and will provide a copy of the report.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection conducted on March 1, 2018, the licensing inspector observed a broken paper towel holder and a broken toilet paper dispenser in Room 2. Plan of Correction: A work order will be placed and a picture will be taken of the broken items.
Standard #: 22VAC40-185-510-C Description: Based on observation and inspection of the facility, the licensee did not follow the center's procedures for regarding methods to prevent use of outdated medication. Evidence: 1. During the inspection conducted on March 1, 2018, in Room 2, the licensing inspector observed an emergency medication in a bag that expired in December 2017. 2. The center's policy for outdated medication indicated that "medication must always be returned to the parent when medication has expired, has been discontinued or if the consent has expired. If you are unable to return the medication to the parent within 14 days, follow these guidelines: Take the medication out of its original container; Mix the medication with an undesirable substance; Place the material in a leak proof container; Throw the container in the trash; Medication administration items, such as expired epinephrine auto-injectors must be disposed of in a closed impenetrable container. Plan of Correction: The parent will be contacted to pick up the medication immediately.
Standard #: 22VAC40-185-560-F Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks there shall be a menu of foods served in the current one week period and the menu must be dated. Evidence: During the inspection conducted on March 1, 2018, the posted menu was not dated for the current one week period. Plan of Correction: The menu was dated during the inspection.
