Comments:
On 2/28/2018 a self-reported incident regarding care and protection was self-reported. An unannounced focused investigation was conducted today between the hours of 10:00am and 10:32am. Based on the preponderance of evidence gathered, violations were cited. The findings from today's visit can be found on the Violation Notice. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview, the center failed to provide care and protection for one child. Evidence: On 2/28/2018, staff #1 did not follow the centers bottle verification procedures and fed a bottle of breast milk that was labeled for child #1 to child #2. Staff #2 noticed that child #2 was drinking from the wrong bottle and immediately took the bottle from the infants mouth. Center staff estimates that child #2 drank approximately 1 ounce of child #1's breast milk bottle.

Plan of Correction: The staff member has been terminated. We have a very strict way of feeding the babies and the staff was trained on bottle verification on 2/19.
