An unannounced monitoring inspection was conducted on March 6, 2018 from approximately 10:00 am to 1:30 pm. Nine staff were present with sixty-eight children. Six children's and six staff records were reviewed. Children were observed engaged in various learning activities in the classrooms.
Standard #: 22VAC40-185-550-L Description: Based on review, facility did not ensure parents were notified the same day when a child sustained an injury. Evidence: On March 6, 2018, the record for child #4 had documentation that on two occasions the child sustained an injury and the parents were not notified on the day it occurred. Plan of Correction: Staff will be trained on March 10th to reiterate documenting the date parent was told about the injury regardless whether the parents signs the incident report that day.
Standard #: 22VAC40-191-60-B Description: Based on review, facility did not ensure 1 of 6 staff complete the required sworn statement or affirmation prior to beginning employment. Evidence: On March 6, 2018, the record for staff #3 (start date February 27, 2018) does not have a completed sworn statement or affirmation form that went into effect July 1, 2017. Plan of Correction: The new form was printed off during inspection and the staff will complete the updated form today.
