The licensing inspector conducted an unannounced complaint inspection in response to a complaint regarding supervision that was received by the licensing department on 02/16/2018. The inspection was initiated and completed on 02/23/2018. On 02/23/2018, the inspector was on site from 10:10AM to 11:30AM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Description: Based on observations, documentation review and interviews, the facility did not ensure the required staff-to-child ratio of 1:5 was maintained for children 16 months to two years of age. Evidence: 1. The licensing inspector arrived at the facility on 02/23/2018 at 10:10AM to address a complaint received by the department. 2. At 10:14AM, the licensing inspector observed Staff #1 exiting the gym, coming down the hallway and entering the Early Head Start 1 classroom by herself with seven children. 3. The licensing inspector entered the Head Start I classroom and reviewed the daily attendance sheet. Seven children were signed in with the youngest child being Child #1 (20 months of age). 4. The licensing inspector asked Staff #1 if she was by herself. Staff #1 stated she was not, that Staff #2 had been with her in the gym. The licensing inspector asked Staff #1 if she knew where Staff #2 was now, and Staff #1 stated she did not know. 5. The licensing inspector walked down the hallway and observed Staff #2 in the Preschool I classroom (Infants - 2 year olds). Staff #2 was assisting Staff #3 supervise eight children. 6. At 10:17AM, the licensing inspector returned to the Early Head Start I classroom. Staff #1 was observed as the only staff member in the classroom, supervising seven children. 7. At 10:26AM, the licensing inspector observed Staff #4 enter the Early Head Start I classroom to assist Staff #1. The licensing inspector asked Staff #4 if she was assigned to the classroom. Staff #4 stated she had been present at the facility until 10:00AM. Staff #4 stated she had left the facility to go work at another location from 10:00AM until 10:20AM, until she was told she was needed back at the facility.
