Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(2)-60-A (Children's records22VAC40-185-(4)-280-B (Hazardous substances); ); 22VAC40-185-(8)-560 (Nutrition);

Comments:
An unannounced monitoring inspection was conducted on 3/6/18 from 3:00pm - 5:00pm. During the inspection, there were 127 school age children in care with 13 staff. Children were observed participating in various activities in the classrooms, playing in the gym, doing homework, and eating dinner. Records were reviewed for ten children while at the facility. The records for ten staff were reviewed on 3/7/18 at the central office. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. There was peeling paint on the inside of the gym doors. 2. There is a piece metal used to hold large pads on the wall in the gym that is bent and has a sharp edge. 3. Staff #11 (Program Director) confirmed that the items listed above were not currently in a safe and operable condition.

Plan of Correction: The facility responded: A work order has been placed to have the doors painted.
