Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.
The following inspection took place between the hours of 2 pm through 4 pm. Today, I met with the Staff of the YMCA after-school program located on site at Cool Spring Elementary school in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 32 children with 3 staff; good ratio for this 1 group. A total of 5 children?s files were reviewed. Staff files shall be reviewed off site on a later date. The children were engaged in the following: check-in, learning about the great wall of China, large motor skilled activities in the gym, snack, homework, and other organized activities. Standard #: 22VAC40-185-130-A Description: Based on review, 1 of 5 children's files reviewed indicated that the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence - 1. On the date of inspection (03/13/2018), the immunization page for Child #1 was not available for review. 2. Child #1 was still enrolled on the date of this inspection. 3. While the immunization page may be on file in the school office, immediate access of this documentation during program hours was not guaranteed. Plan of Correction: A copy of the current immunization page for Child #1 shall be obtained and placed on file.
Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 5 children's files reviewed indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after the child's attendance. Evidence - 1. On the date of inspection (03/13/2018), documentation of a physical examination for Child #2 was not available for review. 2. The start date for Child #2 was listed as 08/29/2017. 3. While documentation of a physical examination may be on file in the school office, immediate access of this documentation during program hours was not guaranteed. Plan of Correction: A copy of the current physical for Child #2 shall be obtained and placed on file.
Standard #: 22VAC40-185-510-A Description: Based on review, the authorization of file for medication to be administered on an as needed basis was out dated. Evidence - 1. On the date of inspection (03/13/2018), a medication authorization for Child #6 has the duration dates as school year 2016 - 2017. 2. Child #6 was in care on the date of inspection. Plan of Correction: The Parent of Child #6 will be asked to obtain a new medication authorization.
