Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on March 7, 2018 from 2:00 p.m. through 5:30 p.m. Upon arrival, there were seventy-one children in the care of six staff. Six children's and six staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Areas observed were: arrival, hand washing/bathroom procedures, outside play, snack, homework and free activity. Interactions between the children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-185-560-N Description: Based on observation, center staff failed to ensure two children were not seated while eating food. Evidence: 1. After snack time for the third grader group, two children were observed walking around and not seated while eating a lollipop. 2. Two third grade children were observed playing outside on the playground while eating a lollipop. 3. The site director confirmed the children had lollipops. Plan of Correction: Moving forward, we will ensure that any child having a lollipop/food shall be seated while consuming it. Any child consuming food/lollipop shall remain seated with another staff if their group moves to another area.
Standard #: 22VAC40-191-60-C-1 Description: Based on record review, the center failed to obtain a criminal history record report with 30 days of hire for two staff employees. Evidence: 1. Six staff records were reviewed. Staff # 1 and Staff #2 did not have documentation of a criminal history record report within 30 days of employment. 3. The hire date for Staff #1 and Staff #2 were documented as 12/15/2017. 4. The site director confirmed the criminal history record reports had not been obtained for Staff #1 and Staff #2. Plan of Correction: Unit directors will submit their hiring packages to the Director of Operations. The Director of Operations will review all of this documentation and then send out background checks. The Director will document the date it goes out and the date it comes back. Staff will not be employed until eligible fingerprinting checks have been received and reviewed.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, center staff failed to ensure a central registry finding was completed within 30 days of hire for two employees. Evidence: 1. Six staff records were reviewed. 2. Staff #1 and Staff #2 did not have documentation of a central registry finding report being requested within 30 days of employment. 3. The date of hire of Staff #1 and Staff #2 was documented on 12/15/2017. Plan of Correction: Unit directors will submit their hiring packages to the Director of Operations. The Director of Operations will review all of this documentation and send background checks to Central Registry. The Director will document the date it goes out and the date it comes back.
