An annual subsidy health and safety inspection (SHSI) was conducted from 3:50 p.m. to 5:45 p.m. with the director and three staff. Five staff records were reviewed including staff training and CPR and first aid information. SHSI requirements were reviewed. Standard #: VENDSUB-000-006-B Description: Based on review of six children's records it was determined that the center did not ensure an action plan was on file for each child with a documented food allergy. Evidence: Child A has documented food allergies. There was no documentation on file indicating action to take if the children come into contact with the specific food allergen. Plan of Correction: The Site Director followed up with the parent to request that the doctor include more specific information on the allergy action plan to include specific actions to take if the child comes in contact with the allergen, rather than just if anaphylaxis occurs.
Standard #: VENDSUB-000-012-E-1 Description: Based on review of documentation, the center failed to ensure staff maintained current CPR certification. Evidence: Staff D's most recent CPR certification was dated 1/18/2016 and expired on 1/18/2018. Plan of Correction: The Site Director will maintain a spreadsheet of important dates to ensure that staff do not expire before getting renewed in the future. We have reached out to the trainer to schedule a class as soon as possible for the staff member to attend.
