Technical Assistance:
1. Reviewed written attendance record requirements. 2. Information about new subsidy regulations. Discussed proof of identity.

12:10-1:15pm Conducted an unannounced annual subsidy inspection. There were no children present during the inspection. Met with the vendor and adult #1. The home was found to be very clean, organized, and sufficiently supplied with toys for the children. Recordkeeping was well organized. The vendor met the annual child development training requirements, and is currently certified in First Aid/CPR. Recordkeeping violation found.

Description: Based on records reviewed, it was determined that immunizations were not updated every 6 months until the child turns 2 years old. Evidence: 1. Child #3 - Immunizations were dated September 2014, when the child was 21 months old. 2. Child #4 - Immunizations were dated October 2017, when the child was 15 months old.

Plan of Correction: The vendor said she will ask the parent for updated immunizations.
