Comments:
An unannounced monitoring inspection was conducted from 9am through 11:00am. There were 10 children in care with three staff. Children present ranged in age from 2years to four years old. Children were observed in teacher and self-directed activities including breakfast, story time, outside play, restroom routines, hand washing and circle time. Violations regarding meal service, background checks, tuberculosis screening, immunizations and physicals were observed and appear on the violation notice.

Standard #: 22VAC40-185-130-A
Description: Based on record review and interview the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child may attend the center. Evidence: 1 - The record for child 2 does not contain documentation of immunizations. a. Child 2 has a documented first date of attendance as 2/10/16 2 - 2 - Staff 1 confirmed that there is not documentation of immunizations in the record of child 2.

Plan of Correction: Center management will ensure that each child has the required immunization documentation on file before the child may attend the center.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview the center failed to ensure that each child has a physical examination. Evidence: 1 - The record for child 2 does not contain documentation of a physical examination. a. Child 2 has a documented first date of attendance as 2/10/16 2 - Staff 1 confirmed that there is not documentation of physical in the record of child 2.

Plan of Correction: Center management will ensure that all children's records contain all required information within the allotted time frames.

Standard #: 22VAC40-185-160-A
Description: Based on record review and interview the center failed to ensure that each staff has a negative tuberculosis screening within 21 days of employment or 12 months prior to employment. Evidence: 1 - The record for staff 3 does not contain any documentation of a tuberculosis screening. a. Staff 3 has a documented date of hire as 10/3/17. 2 - The record for staff 4 contains documentation of a tuberculosis test that is dated 7/1/15. The documented date of hire for staff 4 is 6/20/16. Therefore, more than 12 months had past and the test does not meet the required standard. 3 - Staff 1 confirmed that there is not a tuberculosis screening on file for staff 3 nor a does the tuberculosis test on file for staff 4 meet the requirement.

Plan of Correction: Center management will ensure that each staff has a tuberculosis screening that is obtained with the required time frames.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's record contains all required information. Evidence: 1 - The record for child 1 does not contain addresses for the emergency contacts listed. a. Child 1 has a documented date of enrollment as 1/2/18. 2 - Staff 1 confirmed that the record for child 1 does not include addresses for the emergency contacts listed.

Plan of Correction: Center management will ensure that all children's records contain all required information within the allotted time frames.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview the center failed to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1 - The breakfast served to children in care did not include a fruit. 2 - Staff 1 confirmed that there was not a fruit was not served with breakfast

Plan of Correction: Center management will ensure that any staff preparing meals or snacks is trained in the required components.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview the center failed to obtain required fingerprint. Evidence: 1 - The record for staff 2 does not contain any finger print results. 2 - While the documented date of hire is 11/20/17 Staff 1 stated the background check was resubmitted after January 22, 2018 after which a fingerprint was required.

Plan of Correction: Going forward center management will ensure that fingerprint results are requested and obtained for all new hires as required.
