Consultation provided on attendance accurately reflecting the children present in each group at any given time. Make sure you are keeping copies of the injury/accident reports. Please mark the playground off with caution tape and sinage to make sure it is clear to all that the equipment is not being used.
An unannounced monitoring inspection was conducted today with the director, who were present with two additional staff and 28 napping children. There were no medication on site. Four staff and six children's files were reviewed. For lunch, the children were provided meatball submarines with cheese, green beans, applesauce and milk. Arrival time: 1:35 pm Departure time: 3: 35 pm
Standard #: 22VAC40-185-330-B Description: Based on observation, it was determined that the climbing structures on the playground did not have at least 6 inches of resilient surfacing underneath and surrounding the structures nor were their adequate fall zones of 6 feet of clearance for these structures. Evidence: The mulch surrounding and underneath the 10 climbing and sliding structures had degraded and decomposed to dirt resulting in less than an inch of resilient surfacing ( mulch), the standard requires at least 6 inches of resilient surface. The fall zone of the ten pieces of the equipment were within 6 feet of the other equipment, or the grass boarder or a tree. A fall zone must be free of hazardous obstacles such as other equipment, trees etc. Plan of Correction: We will place these pieces of equipment off limits until brought into full compliance.
