Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: Room 5 and room 6 had cleaners in a cabinet approximately 5 feet off the ground. The cabinet in room 5 was locked with a key but the door was not shut properly to allow the lock to keep the door closed. The cabinet in room 6 was not locked closed.

Plan of Correction: During the manager walks will ensure all cabinets are locked and closed.
