Discussed mandated reporting. Discussed the center's policies regarding how staff are trained to handle stressful situations.
On-site to conduct an inspection regarding an allegation that was received at our office on 2/2/2018 regarding a staff member's handling of a child. Investigation took place between the hours of 2:00 pm and 5:35 pm. Based on the preponderance of evidence gathered from staff interviews, the center's video recordings and written documentation, the allegation is not valid however other violations were cited during this investigation. Standard #: 22VAC40-185-370-4 Complaint related: No Description: Based on review and observation of center's video recording, one staff member failed to provide comfort as needed. Evidence: On the morning of 12/29/2017 at approximately 7:40am, Child #1 (12 months of age) was dropped off in the Toddler B classroom to Staff #1. Child #1 was crying at the time of drop off and continued to cry after their parent left the classroom. Staff #1 placed Child #1 on the carpet and proceeded to conduct a daily health check on another child in the classroom while Child #1 continued to cry. Staff #1 was observed shushing Child #1 from several feet away while the child cried uncontrollably for 6 minutes. Staff #1 did not attempt to soothe or comfort Child #1. Plan of Correction: We did more training on how to administrate proper daily health checks and reminded everyone to utilize the phones for more support if needed.
Standard #: 22VAC40-185-500-A Complaint related: No Description: Based on center's video recording, children's hands were not washed before snacks. Evidence: The children in the Toddler B classroom did not wash their hands prior to being served a drink and snack at the table. Plan of Correction: We have signs all over the school now to help emphasize hand washing.
Standard #: 22VAC40-185-500-B Complaint related: No Description: Based on center's video recordings, the diapering surface was not cleaned and sanitized after use. Evidence: Staff #2 did not sanitize the diaper changing surface after changing a child's diaper in the Toddler B classroom. Plan of Correction: We had a training on proper diaper changing.
Standard #: 22VAC40-185-560-N Complaint related: No Description: Based on center's video recording, children were allowed to drink while walking around. Evidence: Children in the Toddler B classroom were permitted to wander around the room while drinking from their sippy cups. Plan of Correction: We will make sure everyone is sitting while eating and drinking.
Standard #: 22VAC40-191-60-B Complaint related: No Description: Based on record review, it was not verified if an employee has lived outside of the state of Virginia within the last five years. Evidence: Staff #1 completed a sworn disclosure statement on their date of hire, 8/1/2017. The sworn disclosure statement which was filled out was an expired form that does not ask them employee if they have lived outside of the state of Virginia within the last five years. It has been required as of 7/1/2017, that a center must verify whether or not an employee has lived outside of the state of Virginia within the last five years. Plan of Correction: She has a new form on record now.
