The licensing inspector conducted an unannounced renewal inspection Monday, March 26, 2018 from approximately 10:00AM to 1:50PM. The following ratios were observed with a total of 66 children present during the inspection: Infants - 12 children with 3 staff Toddlers - 14 children with 3 staff Twos - 16 children with 2 staff Threes - 14 children with 2 staff Pre-K - 10 children with 1 staff School age children were not present during the inspection. The children and staff were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing on the floor with their teachers, being held, rocked and fed, having books and songs sang to the babies. The Toddlers were observed playing outside. The Twos were observed having their diapers changed and learning about lions during circle time. The Threes children were observed looking at books and participating in circle time. The Pre-K children were observed playing outside and eating lunch. Lunch consisted of black beans and cheese tortillas, peas and carrots, applesauce and milk. Later the children were observed resting at naptime. All areas of the facility including classrooms, hallways, playgrounds, bathrooms and playgrounds were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, injury prevention plan, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. Liability insurance expiration is 12/1/18. The center's first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Last documented health inspection: 05/19/2017 Last documented fire inspection: 02/09/2018 During the inspection, seven children's records and eight staff records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. The renewal application was received prior to the expiration of the current license. Standard #: 22VAC40-185-60-A Description: Based on review of seven children's records, three records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the following: 1. The record for Child #1 (start date 03/26/18) did not contain the parent agreement regarding reporting communicable diseases. 2. The record for Child #4 (start date 01/5/15) did not contain documentation of annual review. 3. The record for Child #6 (start date 08/3/16) did not contain the addresses for the two listed emergency contacts. Plan of Correction: We will contact the parents and add the missing information.
Standard #: 22VAC40-185-70-A Description: Based on review of eight staff records, three records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the records for Staff #5 (hire date 02/27/18), Staff #6 (hire date 03/13/18) and Staff #7 (hire date 03/08/18) did not contain a designated emergency contact along with address and phone number or information regarding any health concerns that staff might have. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-270-A Description: Based on observations, the facility did not ensure all areas were maintained in a safe and operable condition. During the inspection, the licensing inspector observed the Toddler's classroom door leading to the playground had a piece of metal edging at the bottom that was bent out and protruding approximately three inches. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-290-3 Description: Based on observations, the facility did not ensure all electrical outlets had protective covers. Evidence: During the inspection, the licensing inspector observed two electrical outlets that did not contain covers in the Discovery Preschool classroom and an electrical strip by the television in the Pre-K classroom that did not contain a cover. Plan of Correction: Corrected during the inspection.
