Today's visit was an announced non-mandated monitoring inspection for the Provisional Licensure period. During the inspection the previous violations cited at last inspection. The following was reviewed or observed: diapering, five children's records, the flashlight for the upper building and ratios. The inspection began at 10:00am and concluded at 1:00pm. Standard #: 22VAC40-185-60-A
Description: Based on the record review of five children's records that were cited during the previous inspection, the center failed to ensure a record was corrected with the required information. Evidence: The record for child #1 did not include the physical address for one out of two required emergency contacts.

Plan of Correction: Enrollment for the child was discontinued on 1/27/18.

Standard #: 22VAC40-185-350-E-1
Description: Based on the inspector's observations and staff interviews, the center failed to ensure the staff to child ratio for children birth to the age of 16 months was maintained in the toddler classroom. Evidence: The inspector observed six children with one staff member and one volunteer(#1) in the toddler classroom. The age of the youngest child in care was verified as 15 months of age and the ratio for the age group is one staff member for every four children. The volunteer by definition is prohibited from counting in the staff to child definition. There was not a staff record available for the volunteer to count in the ratio to be able to meet the definition of a staff member.

Plan of Correction: The center will work towards completing a staff record for the staff member. The volunteer will not count in ratio until the record is complete.
