Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the center failed to obtain documentation of the required immunizations for one child before the child began attending the center. Evidence: 1. Child #3's immunization record was received at the center on 11/15/2017. Date of enrollment for Child #3 was 11/13/2018.

Plan of Correction: In the future, the director will ensure that a child's immunization record and documentation that the child is adequately immunized are received before a child can attend the center.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records and verification from the director, the center failed to obtain the required information regarding emergency contacts for two children. Evidence: 1. The record for Child #3 did not contain documentation of the complete address for the second person to contact in case of an emergency and the parent could not be reached. Date of enrollment for Child #3 was 11/13/2017. 2. The record for Child #4 did not contain documentation of complete addresses for two people to contact in case of an emergency and the parent could not be reached. Date of enrollment for Child #4 was 08/23/2017.

Plan of Correction: The director will contact the parents of the children and obtain the missing information.

Standard #: 22VAC40-185-210-A-4
Description: Based on review of five staff records and discussion with the director, the center failed to obtain documentation that two staff members had the required programmatic work experience to be qualified to be Lead Teachers. Evidence: 1. The record for Staff Member #2 did not contain documentation that she had obtained at least six months of programmatic experience before being promoted to a Lead Teacher. Date of promotion was 10/27/2017. 2. The record for Staff Member #4 did not contain documentation that she had obtained at least six months of programmatic experience before being promoted to a Lead Teacher. Date of promotion was 10/26/2017.

Plan of Correction: The director will obtain documentation from previous employers to verify that staff members have the required programmatic experience.

Standard #: 22VAC40-185-450-D
Description: Based on classroom observation, the center failed to ensure that pillows sent into the center from home for children to use at nap time were covered with pillowcases. Evidence: 1. During rest time, three children were using pillows which were not covered with pillowcases.

Plan of Correction: The director will make sure that if pillows are brought into the center they are covered with linens.

Standard #: 22VAC40-185-560-F
Description: Based on review of the posted menu and confirmation from the director, the center failed to post the menu listing foods to be served for snacks during the current one-week period. Evidence: 1. The menu containing a list of the foods being served for the morning and after snacks for the current week was not posted. 2. The director confirmed that the lunch menu for the month was posted but the snack menus had not been posted for the parents.

Plan of Correction: The director will ensure the morning and afternoon snack menus are posted from now on.
