An unannounced monitoring inspection was conducted on March 27, 2018 from approximately 10:35 am to 2:55 pm. Two staff were present with twenty-five children at the onset of inspection. Children were observed playing freely, taking a nature walk, participating in center play, eating and napping. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the program director during the exit interview conducted on this date.

Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: On March 27, 2018, the following was observed: 1. There is a hole approximately 2" x 3" in the wall in the snack room leaving 2 exposes wires, 1 of which is missing part of the rubber covering. 2. A purple plastic tub holding toys in the main childcare room is cracked leaving sharp edges and pinch points. 3. The children were taken outside on a walk around the building. There were goose droppings on the ground of the path taken. Cigarette buds were also observed on the sidewalk.

Plan of Correction: The Center Supervisor has ensured the open hold for cable access is permanently covered. The Center Supervisor has removed and replaced the plastic toy tub that was cracked. The Center Supervisor will ensure the paths walked by children on nature walks are clean and clear.

Description: Based on observation, facility did not ensure electrical outlets shall have protective covers. Evidence: On March 27, 2018, 2 electrical outlets in the girls' bathroom were not covered.

Plan of Correction: The Center Supervisor will ensure all electrical outlets have protective covers.

Standard #: 22VAC40-185-340-F
Description: Based on observation, facility did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: On March 27, 2018 at 11:59 am, child #1 (age 3 years) came into the snack room while the children and staff were in the main childcare room. Child #1 was not in sight supervision of staff for approximately 1 - 2 minutes before she re-entered the classroom on her own. Staff did not acknowledge the child had left the room.

Plan of Correction: The Center Supervisor will ensure staff are not allowing children to move ahead without the group.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation, facility did not ensure there is at least 1 staff for every 10 children when the children are 3 and 4 years old. Evidence: On March 27, 2018 at 10:42 am, 2 staff were present with twenty-five children, ages 3 - 5. A third staff arrived at 11:15 am.

Plan of Correction: The Center Supervisor will notify administration immediately in anticipating of being out of ratios to ensure proper staffing based on number tracking.
